  Medicare Part D Rx plans $53 or $55 for initial Part D plan, $26.50 or $28 if not new, or renewal
     
  Medicare Advantage plans $402 or $403 if new MAPD plan, $201/$201.50 if not new to MAPD
     
Aetna Individual 15% the 1st year, 7% thereafter
  Group 5.65% of premium
     
Assurant aka John Alden or Fortis Individual starts at 8% and decreases down to 1%
  Group 6.5% of premium first year, 4% thereafter
  Short-term 14% of premium
     
BCBS Individual policies effective 3/1/11 and after, or who changed plans after Oct 2010, without association discount year 1: 10% of CEP, then 5% of CEP thereafter
     
CEP = commission eligible premium, which does not include rate-ups, surcharges or rate increases (ie CEP is the lowest 'preferred' rate at initial enrollment, not total or actual premium, and not of premiums after future years' renewal rate increases)
     
  Individual policies effective 3/1/11 and after, or who changed plans after Oct 2010, with association discount year 1: 8% of CEP, then 4% of CEP thereafter

    4.5% for years 7-10
     
    Age 81+ 7.5% of premium years 1-6.
